How to Get Started

Step One: Request an Appointment

Choose an appointment time either in office or via telehealth.

Step Two: Upload Your Insurance Information

Provide your insurance information and upload a copy of our insurance cards. We will verify your coverage for nutrition counseling.

Step Three: Complete Intake Paperwork

After we confirm your appointment, you will receive intake paperwork via email. Please complete at least 24 hours before your appointment.

Don't have insurance? No problem.

Cash pay rates are $185 for a one hour initial assessment and $85 for a 30 minute follow up appointment. Please choose a Self Pay appointment when you request an appointment time.

What to Expect at Your First Appointment

At your first appointment, we will review your health history, any recent lab work, your medications and supplements, as well as your goals for your health.

Your first appointment will be primarily assessment of your health, then we will work together to develop a personalized plan to transform your health. While we will be mostly going over your history, symptoms, and goals at your initial appointment, you will walk away with clear action steps that you can implement right away.

We will also determine your plan for follow-up, whether it be a la carte nutrition counseling sessions or my comprehensive program, Nourish & THRIVE Functional Healing.
